Insurance

Buckeye Dermatology is a provider for most major insurance companies.  The following is a list of many insurance companies with which we participate. While we try to maintain this list and keep it current, if your insurance is not listed, please contact your insurance provider to confirm our office’s participation with your plan.

Insurance copayments are due at the time of service.  Your insurance is a contract between you, your employer, and the insurance company.  To file a claim, you must present a CURRENT copy of your insurance card(s) at each visit and communicate changes in your personal information.  If your plan requires a referral, please make sure you have called your primary care physician before your appointment to obtain it.

Not all services are a covered benefit with your insurance plan, so it’s very important that you understand the provisions of your individual policy.  We cannot guarantee payment of all claims by your insurance company.  Reduction or rejection of your claim does not relieve you of your financial responsibility.
